LASIK Retreatment Plans

At LASIK Benefits USA, we trust our credentialed LASIK surgeons to provide you with the best quality care. As a result of this trust, we provide our Members with a one-year Provider Retreatment Plan at no additional cost.

Annual Renewable Plan – The Annual Renewable Plan assists patients in maintaining their best vision possible with free re-treatments (except for the laser manufacturer’s licensing fee of $110-$300 per eye) when they are medically advisable within the first year of the initial procedure. To extend this Plan after the first year, simply call LASIK Benefits USA to schedule an exam prior to the anniversary of the initial procedure. You may renew your Annual Renewable Plan for an additional twelve months. Provider participation may vary.

Assurance Plan – The Assurance Plan provides the Member with retreatment during the first year after their initial surgery. This is at no cost to the Member for physician services, however the Member will have to pay the laser manufacturer’s licensing fee approximately ($110-$300 per eye). If the Member purchases the Lifetime Assurance Plan from one of our Providers, the Member will obtain no cost retreatments not just for the first year after surgery (including no manufacturer’s licensing fee) but also in subsequent years as long as they are medically advisable. Provider participation may vary.
